Initial Consultation
We appreciate you taking the time to visit us for your initial consultation. We will do our best to make your visit worthwhile and efficient. We look forward to meeting you and taking steps together towards your recovery. We have listed below some important questions and documents to bring to your visit. Visit Timeline
Before you arrive for your visit, we ask that you fill out two patient forms to provide us with some information about the reason for your visit. Please click here to access the forms.
If you have not had X-rays recently and are a new patient, we will likely ask for a new set of X-rays when you arrive for your visit. The Radiology department is located at Ground Floor of the Starr Pavilion and directions will be provided after you check in for your visit.
The initial consultation is an opportunity for you and Dr. Kew to get to know each other and to discuss in detail the reason for your visit. Being able to provide specific details about your symptoms will allow us to discuss the appropriate next steps. Example questions are as follows:
How long have you had symptoms? Was there an acute event that led to your symptoms?
·What makes your symptoms better or worse?
When do you experience the symptoms? Do specific activities exacerbate the symptoms?
Where are the symptoms located?
What activities and sports are important to you? How are your symptoms limiting your participation in these activities?
If you have received prior treatment for these symptoms, please bring a detailed record of prior medications, injections, physical therapy, or surgical procedures. If you have had prior surgery on the symptomatic knee or shoulder, please request a copy of your operative report from your surgeon’s office.
If you have had prior imaging on your symptomatic knee or shoulder, please reach out to the facility where the imaging was performed and obtain a CD with the images, as well as a copy of the imaging report. Imaging can include: X-rays, MRI, or CT. If you have not had X-rays recently, we will likely request new X-rays at your visit.
Finally, please bring a list of questions to ask Dr. Kew about your symptoms. It is normal to feel overwhelmed after a visit and we will do our best to explain your diagnosis, treatment plan, and next steps. If you have further questions, feel free to browse the Education section compiled by Dr. Kew or call the office to discuss specific questions about your treatment plan

Patients Enrolled in Participating Health Plans:
You as the patient will be responsible for all applicable co-payments, co-insurance and deductibles that your plan requires to fulfill payment responsibility. Please make sure you have your insurance card at every visit and if at any time your insurance changes please provide our office with the new insurance information
If your insurance requires a referral from your primary care physician, it is your responsibility to provide our office with that referral. If a referral is not provided, payment is due at the time services are rendered
If your insurance carrier is billed and you are reimbursed directly, you are responsible for endorsing and forwarding all payments
Patients with Non-Participating Plans:
Full payment is due at the time of service
Work Related or Motor Vehicle Injury:
You are responsible for providing all pertinent information regarding your accident and billing information with you. Information such as your insurance claim number, date of injury, description and location of injury as well as guarantors mailing address and telephone number. In you are unable to provide the necessary information; payment in full must be made at the time services are rendered.
